Health and Wellness Benefits

Medical/Vision/Prescription
Employees choose from one of three Aetna PPO Medical Plans: Premium, Standard or High-Deductible Medical. Plans include chiropractic and acupuncture. Vision plan offers eye exams, glasses and contact lens. The prescription plan is a three-tier plan with a mail order option.

Dental

Aetna Passive PPO plan ($1,500 annual maximum), orthodontia coverage for children ($2,000 maximum); selection of own dental provider; however, use of contracted provider will provide better benefits.

Health/Dependent Care Reimbursement Accounts

Set aside up to $5,000 for un-reimbursable healthcare expenses and/or $5,000 for dependent care expenses on a pre-tax basis.

Employee Assistance Program

Comprehensive service providing short-term counseling for a wide variety of personal difficulties and issues. Up to 8 free visits with Magellan.

Wellness Program

Includes annual reimbursement up to $360 for gym reimbursement, in addition to various fitness related items. Discount ($4.00) on weekly premiums after completion of a confidential and voluntary Health Risk assessment.

Insurance/Disabilty

Life Insurance
2 x annual salary (no Maximum); opt for 1 x annual salary and save $2.00 off weekly medical premiums. 100% paid by Company.

Supplemental Life Insurance

Optional election of up to 5 x annual salary ($500,000 maximum), 50% for spouse ($100,000 maximum), and $10,000 for child(ren). Employee paid.

Accidental Death and Dismemberment Policy

3 x annual salary ($350,000 maximum). 100% paid by Company.

Business Travel Insurance

$25,000, 100% paid by Company.

Short Term Disability

An employee who becomes disabled due to pregnancy, injury or illness, may be eligible for up to 6 months of paid short-term disability at 60% or 100% of annual salary depending on service. California employees are required to file for California State Disability to subsidize payments.

Long Term Disability

180-day elimination period. 60% of monthly earnings ($10,000 maximum). 100% paid by Company.

Retirement and Savings

401(k) savings plan
Employees are eligible to participate in the 401(k) Plan administered by Wachovia. Company will match 100% of employee’s deferral up to 4% of eligible compensation.

Time Away

Vacation and Sick leave are accrued on a monthly basis based on hire date.

Paid Holidays

Company generally provides 7 days per year.

Educational Assistance
Provides financial support to employees who want to further their education and enhance their skills. Employees are eligible after one year of continuous service. 80% reimbursement up to a maximum of $4,000 per calendar year is available to cover tuition, books and professional license fees.
